Failed seals at connections. The industry sealed joints with cloth backed duct tape for decades, and that adhesive reliably dries, curls, and releases within a few years, especially in hot attics. Every one of those joints becomes an opening. Mastic and mesh, the correct materials, stay flexible and bonded for the life of the system.

Crushed and kinked flex duct. Flexible duct is cheap, fast, and unforgiving: pinched under storage boxes, sagging between supports, or bent hard around a joist, it strangles airflow to whatever room sits at the end. Disconnected runs. Genuinely common and genuinely dramatic: a supply boot that separated from its register or a flex run that fell off its collar, dumping the entire branch into a crawl space.
